Score from 0 to 10, weighted from six component scores. Insurers with at least 50'000 insured persons, calculated across all premium regions. The order may look different in your region, see below.

The overall score above weights them by how many adults have each option (FOPH: around 56% without accident cover through the insurer; deductible 300 slightly more common than 2'500). Every year we give awards based on the rating: an overall ranking, categories such as “Consistently cheap” or “Strongest reserves”, and the most price-consistent insurer in every canton. Insurers may use the badge freely.

Regional insurers

Smaller insurers with fewer than 50'000 insured persons. They are measured where they are at home: in the canton where they have the most insured persons. Calculated across all regions they would often look worse, because outside their area they have hardly any customers and are rarely cheap there.

Prices and consistency vary a lot between regions. An insurer that leads in Aargau can be expensive in Geneva. Each canton page shows which insurers are consistently cheap there:

The savings model trap

Without five years of cost data, new savings models may start up to 20% below the standard model (Art. 101 KVV). After that, the discount must be backed by real costs. The data show what that means:

Discount versus the same insurer’s standard model, deductible 2'500, median. The same tariffs tracked from launch year to 2027. So if you switch to a new model and stay, you pay a little more each year relative to standard. The “Discount retention” component measures how strongly this happens at each insurer.

Administrative costs: who spends a lot on itself

The FOPH publishes, for every insurer, what it spends per insured person on administering basic insurance: salaries, IT, advertising and commissions. Advertising and broker commissions are shown separately. The figure does not count towards the score because it is already in the price, but it shows where premium money stays. The total is still comparable.

Discount retention10%Do new models keep their discount compared with the same insurer’s standard model? Measured on the same tariffs from launch year to today. No loss = 10, 2 points lost per year = 0. Insurers without new models since 2021 are not scored here.
Tariff continuity10%Share of tariffs that continue the following year under the same tariff code. 100% = 10 points, 80% = 0. Dropping or renaming tariffs forces customers to switch.
Reserves10%Solvency ratio according to the FOPH as of 1 January 2026: available reserves relative to the legal minimum. 200% = 10 points, 100% = 0. Thin reserves often come before larger increases.

We use the premiums of all insurers from 2020 to 2027 for adults, with and without accident cover and with deductibles of 300 and 2'500. This gives four separate scores; the overall score weights them by FOPH enrolment figures (With accident cover, deductible 300 24%, With accident cover, deductible 2'500 20%, Without accident cover, deductible 300 30%, Without accident cover, deductible 2'500 26%). Tariffs an insurer renames are followed by name. If a component is missing, its weight is spread over the others. The formula is the same for every insurer, and no insurer pays us anything.

Frequently asked questions

Which health insurer is consistently cheap?

According to our Price Consistency Rating 2027, Concordia, Agrisano and KPT come out best. They are cheap today and were in previous years too. Your canton’s page shows which insurer leads in your region.

Why is today’s cheapest insurer often not the best choice?

Some insurers are cheap for one year and then raise prices more than average. New savings models start with a big discount and lose it in the following years. If you don’t want to switch every year, a consistently cheap insurer serves you better.

Do customer reviews count in the rating?

No. The rating relies only on hard figures from the Federal Office of Public Health: premiums since 2020, solvency ratios and supervisory data. Every number can be recalculated.

Do insurers pay for a good score?

No. abovergleich.com takes no commissions from health insurers. The score comes from a fixed formula, published here.
